Eligibility
All full-time W-2 employees are eligible to participate in the DaBella benefits program after 60 days of employment.
Eligible Dependents
When you enroll yourself in medical, dental, and/ or vision coverage, you may also cover your eligible dependents, including:
- Your legal spouse or qualified domestic partner
- Children under the age of 26, regardless of student, dependency or marital status
- Children named in a Qualified Medical Child Support Order (QMCSO)
- Children who are past the age of 26 and are fully dependent on you for support due to a mental or physical disability and who are indicated as such on your federal tax return
Making Changes After Open Enrollment
You can update your benefits at any time during the year if you experience a “Qualifying Life Event (QLE)” as defined by the IRS. A QLE is a significant life change that allows you to enroll in or modify your health insurance plan outside the annual open enrollment period. These events trigger a Special Enrollment Period, giving you the chance to adjust your health insurance coverage to better suit your new circumstances. If you believe you might qualify for a QLE, it’s important to act quickly and reach out to the Benefits Team for more information at MyBenefits@Dabella.us. You must submit your QLE request within 30 days of the event date to be considered eligible.
Enroll
Enroll using our HR System, Paycom. Instructions are provided in the Online Enrollment Guide Checklist, which can be found in your Checklist Dashboard in Paycom. If you have trouble accessing your Paycom account, contact the HR team.

How to Schedule a Consult
To schedule a consult, visit www.touchcare.com/get-scheduled through the TouchCare app, website, or portal. From there, access the scheduling page, choose the type of consult you need, and select a time that works for you.
